How to get your Nagoya City Library (名古屋市図書館) account and use their website. You don't have to go to distant branches for searching/borrowing books/CDs/DVDs. You can just search on their website and pick them up at your nearest branch. You can borrow up to 6 books and 3 audio/video items.

1. Go to your nearest branch

If you don't have a library card, you have to go to your local library branch and ask a librarian for a 「個人貸出登録申込書」 (Kojin kashidashi touroku moushikomisho) which means registration form. Don't forget to bring your ID.
If you already have a library card, go to your local library and request the same form. (You need to fill it out again to get your temporary web password.) Refer to sample forms page.

2. Get your password

When you submit application form to librarian, you can get your temporary password.

3. Access the website

Access Nagoya City Library website. Actually, they have English page, but it's probably not very helpful. So, this procedure is based on Japanese page.
1): At first, click 「ログインする」, means [Login].
2): You put your library card number in 「共通貸出券番号」.
3): You also put your temporary password in 「パスワード」.
4): Click 「ログイン」.

4. Change your password

The first thing you should do is change your password.
1): Click 「パスワード変更」 (change password) or 「パスワード・メールアドレス管理」 (password and mail address management)」 .
2):Fill your password. 「仮パスワード」 means tentative password and 「新しいパスワード」 means new password.

5. Search books, CDs or DVDs.

You can put book title or aurther name or keyword in the blank next to 「簡単検索」 (simple search) and click 「検索」 (search).
You also search more specifically with click 「詳細検索へ」.
You can put keywords in blanks (It doesn't matter if you fill in the keyword one blank or several blanks).
Reference:
書名: Title
著者名: Author
出版社: Publisher
件名: General
分類: ISBN or code number
出版年月: Publish year and month
After fill in the blanks, click 「検索」 (search).

6. Choose and click what you want to borrow.

Click 「予約カートに入れる」 (add to cart).

7. Choose your nearest branch

1): Click drop-down menu next to 「受取場所」 (location) and click 「図書館」 (library). Refere to [Nagoya City Library Branches].
2): Click drop-down menu next to 「場所詳細」 (specific library) and choose your nearest branch.
3): 「メール」 means you will receive an e-mail message when the materials arrive your requested place.
4): Click 「予約申込み」 (put on hold - reservation).
5): Click OK and done.
6): Click ログアウト (log out).

8. Pick them up

You will receive confirmation e-mail about your request. Wait few days or months (it depends). After you get a notification of their arrival, go to your requested place and pick them up.

9. Check on your account page

You can check your due date on 「貸出状況一覧」 (your condition) page and extend your due date if it is available. You can click 「延長」 (extend) to extend. Also see your status on 「予約状況一覧」(status of booking) page.
Usually, the loan period is 2 weeks. Then extension time if from the date you request the extension. (It is not based on the original due date.)
